Pre-checks

Please download and complete each of the following Forms for every applicant aged 18 years and older

1. Complete the Consent to Exchange of Criminal Record Check Information form (for NZ citizens only)
2. Complete the Authority to Release Information (for NZ citizens only)
3. Complete the Form 1195 – Identity Declaration Form Note: You will need a suitable witness to sign this form
   • Must be an Australian citizen, and
   • Known you for at least one year, and
   • Is currently working in a profession or occupation listed on pages 1 & 2, and
   • Is not related to you by birth, marriage or de facto relationship, and
   • Is easy to contact by telephone during normal working hours
   • Sign the correct section applicable to the applicant at Question 4.
4.  Please provide a photograph of each applicant, to be signed by the same suitable witness
   • Passport sized photograph
   • Head and shoulders against a blank background
5. If required, please complete the Form 956 – Please complete question 13 and 24 only.
